WELCOME
Each year, every returning LPS student must have a Health Update Form completed by their parents. This form also gives consent for the nurse to administer Tylenol. If your child will be taking medication other than Tylenol at school, please also complete a Medication Permission Form. If your child has a medical diagnosis of Asthma, Diabetes, Seizures, or a Severe Food Allergy please complete an Action Plan and obtain a physician signature. All forms are linked under the health and medications tab to the left.
INCOMING SENIORS
Prior to starting their senior year all students must turn in proof of the required Meningococcal Conjugate (MCV4) immunization received after their 16th birthday to the school nurse. Proof of immunization may either be faxed or emailed to the nurse. If you are unsure if your student still needs this vaccine or needs to turn in proof of vaccine please call the school nurse.
